No, McKinsey likes to hire PhDs. If you don't have an MBA, you might not get an offer through the campus recruiting process, but they have experienced hire program. (Their website should have more details)
I don't know if she had to start as an Associate though, Associate is the entry level for fresh MBAs and PhDs.
The job market for consultants was really tough in 2003, it was pretty impressive she got an offer that year.
