After 10 years in Sales, I'd say #5 and #4 are very common, to be expected.
#3 exists, but experienced sales managers don't really do it, unless they want to gamble their career away. Seasoned sales managers sandbag, VERY seasoned sales managers know their pipeline at least 1 year out so there's no need to sandbag.
#2 is pretty bad but also somewhat common, typically only happens in a startup company, though. Established companies have audit rules that prevent this from happening. Also in established companies it's very hard to do 500%, so there's no need for them to dock your commission.
As for #1, in a professional environment, if anyone insults you, you should either fight back or quit. I used to know a woman who's a second degree black belt in Tae Kwon Do, nobody dares to fxxx with her. Never take any crap. It's better to be an asshole / bitch than to be a carpet.
